Electronic Patient Engagement- MCH provides electronic beneficiary engagement and completion of the assessments delivered electronically to your patients via email and text/SMS. These assessments are designed to get relevant health information and engage with your patients in between visits, as well as getting updated contact information as part of the process. This information is also helpful to identify patients and prompt them to come into the office prior to next scheduled visit, thus helping to minimize urgent care or ER visits potentially.

E-Visits - Billable for existing patients and leverages Store and Forward technology to gather information delivered automatically through the MCH platform on behalf of the provider. Specific types of assessments are sent to the patient for completion including Health Risk Assessments, Social Determinants of Health assessments, and others. The patient is also asked to confirm and consent to the provider's review of the assessment results.

Annual Wellness Visit (AWV Subsequent) - These visits are mostly administrative and are higher-cost and more time-consuming than a virtual visit and r eview of assessments and medications with Management Central Health providers.

Chronic Care Management (CCM) - Required by CMS for Primary Care First patients and is a necessary service to engage patients with 2 or more chronic conditions. MCH partners with best-in-class companies that provide CCM. Engagement rates are high based on electronic and telephonic outreach to patients.

Complex Case/Care Management - A complex Case Manager Nurse facilitates the care of patients with complex issues such as mental health or substance abuse. Communicates with the patient's other care providers in more complex situations requiring case management intervention.

Remote Patient Monitoring (RPM) - RPM services are valuable for high-risk patients with chronic conditions to monitor, triage, and send to physicians before an emergent event becomes necessary.

Transitional Care (TCM) - Transitional Care Management services address the hand-off period between the inpatient and community settings. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy.

Cognitive Assessment - A comprehensive evaluation of a new or existing patient, who exhibits signs and/or symptoms of cognitive impairment, is required to establish or confirm a diagnosis, etiology, and severity for the condition.

RAF Score verification Utilizing CareLync - CareLync provides a patient facing program utilizing our proprietary Algorithm-based technology to validate and document applicable diagnosis for Medicare beneficiaries. This process has been developed for launch in 2023. CareLync provides patient outreach, scheduling, review with patient, reporting, and billing for no upfront cost. Charged for each completed comprehensive review/encounter.

Other Services - MCH white-labeled patient-facing application allows for other services to be scheduled for on-site visits such as various labs, EKG, ABI, Opioid Assessments, and more.
